You must provide a copy of the following documents for Home Repairs:

Identification for each member of household/Social Security Cards

Proof of Income for all employed members

 -Deed and/or Title to mobile home

​

 -Paid tax receipt for current tax year

​

 -Insurance Policy if applicable

​

 -Bank Statements

​

 - Mobile Home Information if applicable

​

 -Mortgage Statement if applicable

 

Failure to provide these documents will delay the processing of your application.
